For people named on a license

Your name on a license

We show the people the state lists on each license. You can ask us to take your name off our pages, or tell us something is wrong.

  1. 01Send the form. We reply by email within 30 days, usually sooner.
  2. 02To remove a name, we hide it from the profile and from search on our site. The license itself stays, because the business is still licensed.
  3. 03If a detail is wrong in the state file, only the board can fix it. We'll point you to the right CSLB or DBPR form, and our page follows the next file.
